2012 Physicals

Physicals for athletes/activities will be held on June 2, 2012 at Good Samaritan North 900 North Main Street, or on June 4, 2012 at Good Samaritan’s Huber Heights location at 6521 Good Samaritan Way on Brandt Pike. Take with you both the $15 fee and the required and filled-out OHSAA Form which is available at cjeagles.org on the athletics page.
